This part serves as notification to this Department
of the item listed. If there are changes in the item
listed, please complete the relevant item as
stated.

whether existing or newly appointed.
Inform this Department immediately in writing if
there is any change of appointment of authorized
representative after tax return is filed. ITP account
holder may notify this Department about the change of
authorized representative under ‘Manage Service Agent’
service.
